REQUIREMENTS:
- Active QC E-Services account
- If the request is made on behalf of a company, the following additional documents are required:
- Special Power of Attorney
- Board Resolution
- Authorization Letter
STEPS:
- Log in via your QC E-Services account.
- Once inside the QC E-Services, choose ‘People’s Corner’.
- From the options, select ‘Request FOI’.
- Fill out the form with the necessary details.
- Choose the department to which you want to send the request.
- Submit the request.
- Await a response from the city government.
