1. Download the VaxCertPH app or visit www.vaxcert.doh.gov.ph.
2. Fill-in required information to get status.
   • For individuals, with available vax certificate, download document and bring upon departure.
   • For individuals with vax certificates not yet ready, please check again after 48 hours to allow system to update.
     • If you need to change information in the website or the app, please prepare the following:
       • LGU vaccination card
       • passport
       • any government-issued ID
       • Leave your name, email and cellphone number and wait for 3 days to update your vaccine record.

1. you may proceed to your LGU for assistance.  Bring your LGU vaccination card and passport or any government-issued ID.
2. If necessary, you may call 88-7614-88 or email vaxcertsupport.doh.gov.ph.
