How to Apply for Right to Care Card

Quezon City Departments or Offices involved:
GENDER AND DEVELOPMENT COUNCIL OFFICE

REQUIREMENTS FOR ORIENTATION:

  • Available to Quezon City residents who are queer couples, or if at least one partner resides in Quezon City
  • QC ID for QC residents, valid ID for non-QC residents

STEPS:

1. As a queer couple, you have three options to register: manually by visiting the Gender and Development (GAD) Council Office via online registration Right to Care Card Orientation (google.com) or by scanning the QR code.

2. You will be required to attend the orientation on the date indicated in the registration. Please bring your QC ID if you are a QC resident or one valid ID if you are a non-resident.

3. Following the orientation, you both will be requested to sign a Special Power of Attorney (SPA) in the presence of the legal counsel.

4. You will receive a notification once your Right to Care Card is ready for collection.
