REQUIREMENTS

A. SENIOR CITIZEN

1. Present OSCA ID preferably QCitizen ID
2. Photocopy of OSCA ID

B. AUTHORIZED REPRESENTATIVE

1. Present OSCA ID preferably QCitizen ID of Senior Citizen
2. Photocopy of OSCA ID (with 3 signatures and/or thumb mark)
3. Authorization letter signed by Beneficiary, Representative, SSDD/OSCA
4. Present Government Issued ID of Representative
5. Photocopy of Government issued ID of Representative
6. Picture of Beneficiary together with Authorized Representative holding a newspaper (Dated at least 1-week prior the schedule of Cash Pay-out)
7. Barangay Certification (Certifying the relationship of Beneficiary and Authorized Representative)

C. DECEASED BENEFICIARY

1. Present OSCA ID preferably QCitizen ID of Senior Citizen
2. Photocopy of OSCA ID
3. PSA of CTC Death Certificate of Beneficiary
4. Warranty and Release from liability (DSWD provided)
5. Present a Government issued ID of Representative
6. Photocopy of Government-Issued ID of Representative
7. Barangay Certification (Certifying the relationship of Beneficiary and Authorized Representative
