STEP 1: REGISTER ACCOUNT

WITHOUT QC E-SERVICES ACCOUNT

1. Visit the QC Vax Easy website and click ‘Register for a Vaccination’.
2. Click ‘Continue without a QC E-Services’.

WITH QC E-SERVICES ACCOUNT

1. Log-in to your QC E-Services Account and click ‘QC Vax Easy’ from the menu.

STEP 2: FILL UP

1. Fill up the registration form and answer all requested information. Please make sure all answers are correct to avoid complications.
   
   For the Priority Group, choose the priority group where you belong:
   • A1 – Medical Frontliners
   • A2 – Senior Citizen
   • A3 – People Living with Comorbidities

STEP 3: BOOK VACCINATION

1. After submission,  proceed to ‘Book Vaccination’ and choose option ‘Booster’.
2. This will show boosters available for your group, and if you are qualified.

STEP 4: SCHEDULE

1. Choose the Calendar icon at the right side to show all available dates.
2. Choose a vaccination site and time,  then submit.
3. Get a photo or screenshot of your reference number. You will need this to access again your QC Vax Easy Account.
