• 1 photocopy of the Marriage Certificate of the parent.
• 1 photocopy of passport, if one or both parents are foreigners.

Requirements For Unmarried Parents (Illegitimate Child):

1. 4 fully accomplished Certificate of Live Birth Form (Municipal Form No. 102). The hospital, maternity, lying-in clinic, or other birthing facility prepares the Certificate of Live Birth Form.
2. If the father is declared, signed Affidavit of Acknowledgment/Admission of Paternity. This is on the back of the Certificate of Live Birth Form.
3. Signed Affidavit to Use the Surname of the Father. The mother will sign the Affidavit to Use the Surname of the Father.

Process:

1. Submit requirements to the Birth Registration Division of the City Civil Registry Department.
2. Wait to register and get a personal copy of the registered Certificate of Live Birth.

1. Make sure the details on the Certificate of Live Birth Form are correct and complete.
2. On-time registration of Certificate of Live Birth is free of charge.

“Clerical or typographical error” refers to a mistake made in clerical work such as writing, copying, transcribing, or typing an entry on the Birth, Marriage, or Death Certificate of a misspelled name, or misspelling of the place of birth., or anything like it, visible to the eye or obvious to the senses, and maybe corrected or altered only by reference to other existing record or records.

R.A. 9858 

This law is an amendment to the Family Code of Legitimation which allows the civil status of children conceived by parents who are not yet of legal age (minors) to become legitimate children.

Legitimation and R.A. 9858 do not allow the following:

• One or both parents had a first marriage before the birth of the child.
• Child was born without a Court Order Decision of Annulment or Divorce.
• Child was born while the first wife was still alive.

• If the child is not acknowledged by the father in the Certificate of Live Birth, provide a photocopy of the Elementary School Record (form 137/form 138) or photocopy of the Medical Record of the child (immunization or baby book)
• If the child is not acknowledged by the father in the Certificate of Live Birth, Submit a duly signed Admission of Paternity 

Process: 

1. Submit all the necessary requirements to the Legitimation Counter at City Civil Registry Department.
2. Request for Order of Payment and pay the corresponding fee amounting to PHP 500. If an Admission of Paternity is included, payments shall be PHP 850.

Free cremation service at the Baesa Public Crematorium for indigent residents of Quezon City 

Requirements:

1. Cremation Permit was issued by the City Health Department.  
2. Original Barangay Certificate of Indigency

Process:

• Proceed to Social Services Development Department to obtain a schedule for cremation.
• Submit the Death Certificate to the Quezon City Health Department for evaluation and issuance of a Cremation Permit.

• Submit the Cremation Permit and signed Death Certificate to the Death Registration Division for the registration process. 
• Request for Order of payment and pay the corresponding registration fee amounting to PHP 50 at the City Treasurer’s Office. 

• Return to the Social Services Development Department and submit the necessary requirements and receipt from CTO. 
• Proceed to the Baesa Crematorium at the scheduled date of cremation.
• The Urn shall be given after the cremation process. A cremation certificate may be requested at the Civil registry Department.

REMINDER: 

1. Cremation Permit is requested from the City Health Department.  

  2. The Death Certificate should indicate that the Deceased will undergo cremation at the Baesa Public Crematorium.

This is the burial service at the Bagbag Public Cemetery and Novaliches Public Cemetery for those deceased residents of Quezon City. Burial in a public cemetery in Quezon City is only up to 5 years to give other residents the opportunity to be buried in the Quezon City Government’s funeral service.

Requirements:

• Registered Certificate of Death. The Death Certificate should indicate that the Deceased will be buried at Bagbag Public Cemetery or Novaliches Public Cemetery.
• Signed Affidavit of Undertaking to be issued by City Civil Registry Department.
• Burial Permit issued by the Death Registration Division.
• Valid ID (Preferably QC ID) of the Informant.
• Valid ID (Preferably QC ID) of the deceased or Barangay Clearance that will show the residence of the deceased.

Process: 

1.  Upon registration of the Death Certificate, request Burial Permit from Death Registration Division.

2. Request for Order of payment and pay the corresponding fees amounting to PHP 50 (Burial Permit) and PHP 600 (Internment fee-child) or PHP 1,500 (Internment fee-adult) at the City Treasurer’s Office. 

3.  Return the Official Receipt to the Death Registration Division and get the Burial Permit along with the Death Certificate.

4.  Proceed to Bagbag Public Cemetery Office for the scheduling of the Burial service.

5.  Return to the Bagbag Public Cemetery or Novaliches Public Cemetery at the scheduled date of burial.
